A serious and widespread issue has been identified. Within the *past two minutes*, *all patients arriving* are experiencing a *charting bug* where *random clinical orders are being placed into charts* — seemingly without user action or from incorrect/random providers.
Additionally, providers report that *assigned provider information is not appearing* in the patient appointment view, making it unclear who is responsible for the visit.
This is a *live, urgent issue* impacting active patient care.
